Information on Volunteer Hours

Certificates are issued based on annual volunteer hours and will be provided at the end of the year. If you would like to receive your certificate earlier, please submit your request in the comment box.

[How to issue the verification form]

- Verification forms are issued on an annual volunteer hour basis.

- We need about 2 weeks to issue the volunteer hours verification form.

- If you would like to get the verification form by E-mail:
Please submit your request using the "apply your validation form" on your "My Page." We will send back to you the verification form, with an attached file in PDF format.

- If you would like to get the verification form by regular mail:
Please submit your request using the "apply your validation form" on your "My Page." After that, please send us a self-addressed stamped envelope.

- If you need a verification form, please let us know what it will be used for. We will not issue a verification form for a possible future use.
